Salem Endodontic Associates is a Premier Provider for all ODS/Delta Dental plans. As a courtesy, our office will bill your insurance and accept assignment of benefits from your insurance when possible. Please understand that your dental insurance plan is a benefit from your employer to provide a means to partially cover necessary dental treatment costs. We will make every effort to help you understand your coverage when insurance information is provided to us.
Please bring your insurance plan card and/or benefit book with you on the first appointment. Please check with your employer or plan administrator prior to your first appointment to determine if your plan requires utilizing in-network dental providers to maximize your dental benefits.
Once your treatment has been reviewed with you by your doctor, our front office staff will discuss fees and financial options. Our office does require payment at the time of service, and we have extended payment options available through Cherry Payments and CareCredit.
We also accept checks or debit cards, and credit cards such as Visa, MasterCard, Discover and American Express.
